Historically the BCBS plans in which your subscriber ID has the 'R' prefix have covered nutrition counseling under preventive care with no copay. If you are unsure about your specific plan coverage please reach out, I will be happy to check your coverage for you.
Do I need a doctor's referral?
Not all plans require one but it can be helpful to have on file as part of your medical record. You can use the form linked below or your provider can use their own. I am happy to reach out to your provider for you if that feels easier. Referrals should be faxed to Elle @ 866.877.2844
