Payment Frequently Asked Questions

  1. The information I receive from your insurance company does not guarantee coverage. If a claim is denied you will be billed for any completed sessions at the rate billed to your insurance company.

  2. A new patient visit will not be booked until your insurance coverage has been investigated or your new patient package is paid for. Most patients get an initial appointment 1-3 weeks after package payment OR confirmation of benefits from their insurance company.

  3. HSA/FSA cards are accepted. 

  4. Individual session rates can be found here

  5. Meal plan pricing can be found here

  6. If you are not the primary holder of your insurance I will need you to fill out a form with information of the subscriber

  7. If you hold insurance with one of these companies [MVP, BCBS, Cigna] and do not want me to submit claims to your insurance you will be required to sign an insurance opt out form
